Here, licensed Arizona Doctors of Osteopathy will find information on updating the doctor profile and contact information, how to request inactivation of your Arizona license and other information from the Board.
Licensees can also find answers to many frequently asked questions.
Please use our new License Portal to update your address, practice information, specialty certifications, name, or email address. You can also print out wallet cards and certificates for your license. As well as enter your continuing medical information and renew your license during the renewal period. The link to the portal can be found here or in the menu on the right-hand side of the page labeled "Licensee Portal". If you are using a mobile device, click the menu icon and then find "Licensee Portal" under DO Center. We encourage you to start entering your CME information early to avoid potential delays in your renewal process. Please note that licenses are due for renewal by December 31st.

Certification of Vital Statistic Documents in Arizona
The Arizona Department of Health Services Bureau of Vital Records implemented a new electronic death registry system called, the Database Application for Vital Events (DAVE).
