Important Payment Notice!
Effective March 26, 2018, the Arizona Osteopathic Board will only accept credit card payments via mail (USPS, FedEx, UPS, or any other mail carrier). This exception for credit card payments only applies when submitting a paper credit card form as payment. This change is required to conform to the Payment Card Industry (PCI) Compliance Standards. Any credit card payment information received via any other method will be destroyed.

On-line payment transactions will not be affected.
