2018-2019 Arizona Osteopathic Online Renewal

Please read the directions below before starting. The Online Renewal is now compatible with most major browsers. When you are ready to begin, scroll down and click on the link to log in to the Online Renewal system.

License Renewal and CME FAQ

CME FAQ: What is Category 1-A and Category 1?

  1. How long will the process take? The renewal process should take between 10 and 15 minutes to complete.


  2. Check your online profile and verify its accuracy before starting the process. To view your profile, return to the home page and click on "Doctor Search". You will then be able to enter your name or scroll down to enter your Arizona license number. You will have the ability to change your address or phone number(s) while completing your renewal. If you wish to make other changes, please contact the Board. We may need to follow up with you for further documentation. If you wish to add postgraduate training, please send a copy of your certificate of completion to our office by email, fax, or mail.


  3. Payment Method. Visa, MasterCard and American Express are accepted. There is no additional fee to renew online.


  4. CME Audit. If your notice letter says "CME Audit-Yes", you have been randomly selected for CME audit this renewal cycle. You will be asked to input the CME you have completed in 2016 and 2017. Please note only CME completed AFTER your last Renew By Date which was not used to qualify for a previous renewal is valid for this renewal cycle. Please have your CME documents ready in order to enter the data when asked.

If you choose not to renew online, you may print and complete a paper application to send with your check, money order or credit card information. Go to For DOs >License Renewal Forms> License Renewal Application. Renewal applications sent by fax, email or regular mail are processed in the order they are received and can take up to 30 days to process.
